Position Overview: Community Engagement and Wellness Specialist

As a Community Engagement and Wellness Specialist at Meridian Health Plan, you will serve as a vital link between the organization and the communities we support. Your responsibilities will not only include outreach and education but also advocacy and collaboration with various partners to improve health outcomes.

Key Responsibilities

Engage: Connect thoughtfully with community members, ensuring clear communication about available health resources and wellness programs.
Educate: Lead or support workshops, training sessions, and informational events designed to empower community members with knowledge about health management and preventive care.
Collaborate: Work alongside local organizations, healthcare providers, and stakeholders to identify community needs and develop tailored outreach strategies.
Support: Provide ongoing assistance to individuals and families, helping them navigate available services effectively.
Contribute: Assist in data collection and reporting to track program outcomes and inform continuous improvement efforts.
